I Know This Much Is True by Wally Lamb

Two brothers, identical twins, are raised in the 60's by a loving but weak mother and an abusive step father. As the boys grow older one of them starts to fall into a mental illness and never recovers. As adults one brother is forced to come to terms with his twin's illness and personal life choices. Set in a first person narration the well brother comes upon a book written by his grandfather from old Italy. Mid way through the book, it changes into Granpa's story. This is a facinating story of old world culture, mysterious tales, and family history. Meanwhile one brother is kept in a mental hospital for cutting off his hand. This is a wonderful story of brother's love, family history, personal identification, and friendship.
