Thorns of Truth by Eileen Goudge

This sequel to "Garden of Lies" picks up twenty years later when Rose's son announces his engagement to Rachel's emotionally fragile daughter. A worried Rose goes to her mother Sylvie for help. Sylvie is unable to help because it would mean letting the world know that she switched Rose and Rachel at birth, and she is Rachel's adoptive mother and Rose's biological mother.

Meanwhile, the recently widowed Rose must also cope with her husband's loss and the appearence of a new love in her life, as well as her stepdaughter's growing dependance on alcohol. Rachel has her own problems trying to keep her free clinic afloat, and still make time for her husband and daughter. As situations and problems grow and intensify, it soon becomes clear that after all these years Sylvie will have to tell the truth about the switch she made so many years before.
