The female lead is a skilled heart surgeon who begins to doubt her ability to save lives. She is observed and in some ways guided by an angel (male lead) to realize that she has been doing her best and that the deaths which have occurred on her table are not within her control.

In the process of doing this, the angel, from time to time, makes himself visible to the doctor.
In spite of the rules the angel must adhere to, he finds himself falling in love with the doctor and becomes visible to her more frequently.
In a chance contact with a man awaiting surgery, the angel learns that the man is an angel who has chosen to "fall", becoming mortal, thus being able to feel, smell, love.
The angel ultimately decides to "fall" and the doctor (Meg Ryan) and the angel (Nicolas Cage) enter an earthly relationship. He has made the ultimate sacrifice, but their life together lasts only one day as a result of a road accident which kills the doctor.
This story is very much like the dual sacrifices described by O. Henry in "The Gift of the Magi".
