Psychiatrist Issac Barr (Gere) is treating a young woman named Diana Baylor (Thurman). Upon meeting her older sister Heather Evans (Basinger), he falls in love with her and the two begin an affair. Unfortunately Heather is married to a Greek mobster named Jimmy who treats his wife terribly and is a complete tyrant towards her. Issac wants Heather to file for divorce but she is afraid of what her husband might do if she did. Heather has a medical condition that becomes very apparent when she drinks so she is not supposed to but one night her and her husband go out to supper and upon returning home, she physically attacks him, hitting him over the head with a barbell and killing him. Heather is arrested and put in jail. Doctor Barr uses his professional credentials to do what he can for Heather but after many talks with her, and also her sister, he begins to wonder if she is telling him the truth- that Jimmy's death was an accident, brought on by her medical condition, and not premediated first degree murder.
