While inundated with bodies from an accident, Boston Medical Examiner Maura Isles finishes her work and hears something where the bodies are kept. She tries to convince herself it's the refrigerator but it's actually a woman -- alive! Isles calls for an ambulance and they rush the unknown woman into the ER. Things are just as hectic for Detctive Jane Rizzoli. While testifying in court, the pregnant and overdue Rizzoli has her water break and rushes to the hospital to give birth. This is where the stories converge. The woman turns out to be Fierce Olena; a woman bent on getting publicity to bring down the government official who enslaved and murdered her friends. Fierce causes trouble in the hospital causing an evacuation and takes Jane and a bunch of others hostage in the hospital. There is intense negotiation as Rizzoli's husband FBI agent Gabriel Dean tries to find out information and deal with the hostage takers.

Federal agents come in and kill the hostage takers and then all the evidence and bodies leave Boston with them. This doesn't sit right with Jane and she wants to find out the real reason for the federal involvement and while on her maternity leave she investigates and finds out the high level of conspiracy involved.
