The sister of a Japanese ghost terrorizing an American apartment travels to Chicago hoping to end her sister's murderous streak. Naoko is the sister of Kayako, the ghost who has killed many people after she and her son were murdered by her angry husband. Naoko travels to Chicago after learning that Jake, the boy from the second movie (Grudge 2), is killed by Kayako's ghost in a mental hospital. She meets the people in Jake's apartment complex, namely Lisa, her siblings Rose, and the superintendent Max. Soon, some residents in the apartment die, such as one girl who drowns in a tub, and another, a painter, killed by an entity that jumped out of her paintings. Concurrently, Rose, who has a third eye, has been seeing Toshio, Kayako's son, quite often. Meanwhile, Jake's psychiatrist Dr. Sullivan, is being hunted down by Kayako, and is killed when her neck is broken. In the apartment, Max is taken over by Kayako's husband. Naoko, having failed to perform a Japanese exorcism ritual that involves Rose drinking Kayako's blood, is brutally killed by Max. Max then goes after his sisters, but Rose completes the ritual in time, and Max turns back to normal. They think the curse is finally over, only to find out that Naoko is now the new force behind a new curse, following Rose around.

It's a never-ending cycle of killings, repeating the same pattern of scare tactics without using a compelling plot line.

Naoko was a weak character who didn't get the job done, she came to Chicago without a full-proof plan which is stupid.
